Ever heard of an inflamed tendon right on top of the kneecap? This thing hurts like a b****! The pain is there just about all day. I can't kneel down, walk up steps or run a mile without wincing in pain. I've ran 4 25k's and never felt anything like it. Took doc's advice and quit running for about 6 months. It's still not gone and it still hurts. Would surgery be the only option?

You don't seem to have exhausted your conservative options yet. I doubt any surgeon would recommend surgery at this juncture.

(Biased opinion here) Try to get a PT referral, see what the therapist has to offer.
